Letters & Forms Requests

Information for Clients

At Mills Gosse Psychology, we understand that clients may need letters, forms, or documentation for a variety of purposes (e.g., school, workplace, insurance, or other third-party requests). To help us support you as efficiently as possible, please review the following guidelines.


Planning Ahead

If you anticipate needing documentation as part of your care, we strongly encourage you to let us know as early as possible, ideally at the start of services. Early communication helps us:

  • Determine whether your request is appropriate

  • Ensure we have enough time and information to complete it

  • Avoid delays or unmet expectations

Please note that late or unexpected requests may not always be possible to accommodate.


Established Care Requirement

In most cases, we cannot complete forms or letters for brand-new clients. Before providing documentation, your clinician must have a sufficient understanding of your history and current functioning. This typically requires:

  • A minimum of 3 or more sessions, and

  • Sometimes, additional sessions, depending on the complexity of the request


Providing Forms

If you require a form to be completed:

  • Please provide the form in advance for review

  • Forms must be submitted in a fillable electronic format

  • Non-fillable or handwritten forms may result in delays or may not be completed.


When Another Provider May Be More Appropriate

Some forms are specifically intended for physicians (medical doctors) and may not be appropriate for a psychologist to complete. In these cases, it may be more suitable to:

  • Contact your family physician or medical specialist

  • Your clinician can help guide you if you are unsure.


Fees for Documentation

Requests for letters, forms, or reports are completed outside of session time and are billed separately.

  • Our standard rate is $225/hour

  • Fees vary depending on the complexity and time required

  • In some cases, fees may be billed directly to a third party (when applicable)

  • Otherwise, clients are typically responsible for these costs

  • We are happy to provide an estimate where possible.


More In-Depth Requests

Some requests require more detailed clinical information, such as:

  • Diagnosis

  • Functional assessments

  • Disability or work-related opinions

  • Formal evaluations

In these cases, a comprehensive psychological assessment may be required. If this applies to your situation, your clinician will discuss appropriate next steps with you.

Our goal is to provide accurate, thoughtful, and clinically appropriate documentation. Clear communication and adequate time in treatment allow us to do this effectively and in your best interest.
